چکیده

We read with interest the article by Sauleda and colleagues on the use of a simplified exercise test for the initial differential diagnosis of Pneumocystis carinii pneumonia (PCP) in HIV positive patients (February 1994;49:112-4) and would like to make the following points. Firstly, the risk factors for the acquisition of HIV infection were not given other than to state that 36 of the patients were intravenous drug users. It was not clear how these 36 patients divided between the two groups. Intravenous drug use is associated with impaired lung function independently of acquired infection and, in addition, intravenous drug users are more susceptible to pyogenic infections. Of the initial 60 subjects enrolled in the study 25%

Effect of sputum induction on arterial oxygen saturation and spirometry in HIV-infected patients.

We wanted to study the effect of sputum induction on forced expiratory volume in one second (FEV1) and on oxygen saturation in normal controls, asymptomatic human immunodeficiency virus (HIV)-seropositive individuals, and HIV-seropositive patients under investigation for suspected Pneumocystis carinii pneumonia (PCP). Over a five month period, sputum induction with ultrasonically nebulized 3% s...

Diagnosis of Pneumocystis carinii infection in HIV-seropositive patients by identification of P carinii in pleural fluid.

Pneumocystis carinii pneumonia (PCP) is the most common pulmonary complication of AIDS and is typically diagnosed by the identification of P carinii organisms in sputum, bronchoalveolar lavage fluid, or tissue obtained with transbronchial biopsy. We describe two HIV-seropositive patients with pleural effusions in whom the diagnosis of P carinii infection was made by examination of pleural fluid...

Reduced carbon monoxide transfer factor (TLCO) in human immunodeficiency virus type I (HIV-I) infection as a predictor for faster progression to AIDS.

BACKGROUND In addition to the acute fall in carbon monoxide transfer factor (TLCO) associated with Pneumocystis carinii pneumonia (PCP) or other opportunistic lung infections, reduced TLCO occurs in HIV-I seropositive individuals without active pulmonary disease. Abnormal TLCO, in the absence of lung disease, may be a surrogate marker of HIV-I induced immunosuppression and, therefore, a predict...

Quantitative and qualitative comparison of DNA amplification by PCR with immunofluorescence staining for diagnosis of Pneumocystis carinii

Aim: To compare the results of DNA amplification by the polymerase chain reaction (PCR) with immunofluorescence staining for detecting Pneumocystis carin'i in bronchoalveolar lavage specimens taken from symptomatic HIV seropositive patients with suspected P carinii pneumonia (PCP). Methods: Bronchoalveolar lavage specimens were obtained from 28 symptomatic HIV seropositive patients. Specimens w...
